2022/23學年「中一自行分配學位」申請須知
（一） 自行分配學額：40名
（二） 報名日期：2022年1月3日至1月17日。
【辦公時間： 9:00-12:15及14:00-16:30 (星期一至五)、 9:00-12:15 (星期六)
星期日及公眾假期休息】
（三） 於上述日期內，將下列文件親身交回本校一樓校務處（九龍大坑東道12號）：(家長有實際困難而未能親身遞交，可透過郵遞申請。家長需衡量相關風險，截止日期以郵戳日期為準。)
(1) 已填妥的本校申請表
(2) 教育局派發的「中一自行分配學位申請表」
(3) 小五全年及小六上學期成績表 副本
(4) 身份証明文件 副本（香港身份證）
(5) 貼上$2 郵票之長回郵信封(4吋×9吋) 兩個（信封上須清楚書寫學生姓名及地址）
* 如家長以郵遞申請，請貼上$2 郵票之長回郵信封(4吋×9吋) 三個（信封上須清楚書寫學生姓名及地址）
(6) 校內 / 校外的獎狀或證書 副本(如有)
** 不需提交小學推薦信 **
（四） 面試和收生準則：
(1) 面試安排：每位申請者會獲得一次面試機會，面試日期暫定為2022年3月5日及3月12日，面試時間及地點將以郵寄方式通知家長。
** 面試安排可能受疫情影響，請留意教育局和本校宣佈。
(2) 面試形式：以英語及粵語面試、形式包括個人面談及/或小組討論。
(3) 收生準則：
- 申請人操行必須達乙等或以上
- 學業成績及申請學生的成績次第（教育局提供） (70%)
- 面試表現 (20%)
- 課外活動 (10%)
* 在學術、體育、音樂及/或藝術方面有突出成就者將獲額外加分。*
（五） 本校將於2022年3月31日透過郵遞及電話，通知所有正取學生的家長其子女已獲納入本校中一自行分配學位《正取學生名單》。
（六） 獲取錄之學生將於2022年7月教育局公佈中一派位結果時，由教育局直接通知，本校不再另行公佈。
（七） 根據教育局之規定，小六升中一之學生只可向不多於兩所中學申請「自行分配學位」，多報會失去該類學位之申請權利。
（八） 未獲取錄者將繼續依照教育局「統一派位程序」派位，其派位結果不受此項申請影響。
（九） 教育局公佈中一派位結果後，未獲取錄者的所有個人資料及申請表將會被銷毀。
（十） 如有問題，可於辦公時間內致電2380 8837查詢。
Application Guidelines
- Number of places for DP: 40
- Application Period: 3 January 2022 to 17 January 2022
[Office hours: 9:00 – 12:15, 14:00 – 16:00 (Monday – Friday), 9:00 – 12:15 (Saturday), Close on Sundays and Public Holidays] - The completed application form should be submitted to the school office (1/F, Concordia Lutheran School, 12 Tai Hang Tung Road, Kowloon) IN PERSON during the above application period. (Submission BY POST is accepted if parents are unable to hand in the application in person; however, please beware of the application deadline. The cut off time for postal application will be the day of the postmark being stamped.)
Documents to be submitted include:
- A completed Application Form;
- Complete EDB Application Form for S1 Discretionary Places;
- Copies of Primary 5 and 6 school reports;
- A copy of student's Hong Kong Identity Card;
- Two self-addressed envelopes (4”x9”) with $2 postage; (Please write down student’s full name and mailing address on the envelopes.)
(For Application BY POST, THREE self-addressed envelopes (4”x9”) with $2 postage are required.)
- Copies of supporting documents and certificates of participation and performance in extra-curricular activities and services (Recommendation Letters or Reference Letters are NOT to be submitted.)
4. Interviews and Admission Criteria
Arrangements for interviews:
Interviews will be arranged for all applicants. The tentative dates for interviews are 5 March 2022 and 12 March 2022. Details will be informed to parents by post.
**The interviews may be affected due to Covid-19 related circumstances. Please refer to the latest announcements from EDB and the school by then.
Details of interviews:
Languages: English and Cantonese
Format: individual and / or group discussion
Admission Criteria:
Conduct grade must be Grade B or above
Academic Achievements and EDB Rank Order (70%)
Performance in Interview (20%)
Extra-Curricular Activities and Services (10%)
*Bonus marks will be given to applicants who have outstanding performance in academic, sports, music and/or art.
5. Parents of successful discretionary places applicants will be notified by mail and by phone of their children’s
inclusion in the Successful Lists for Discretionary Places.
6. The results of Secondary One Discretionary Places and Secondary School Places Central Allocation will be
released in July 2022 by EDB. Please note that applicants will not be notified individually of their results by
our school.
7. In accordance with the EDB guidelines for Discretionary Places scheme, each applicant can only apply for
no more than two secondary schools under the scheme. Applicants who fail to comply will be disqualified in
the application under the scheme.
8. Unsuccessful applicants will continue the Secondary School Places Allocation through Central Allocation
(CA). The allocation of CA will not be affected by the results of DP.
9. The personal data of unsuccessful applicants will not be retained and will be disposed of after the release of
the allocation by EDB.
